Schedule: Monday–Friday
Guaranteed Hours per Week: 35 Hours
Pay: $40/hr on W2
Job Summary
We are seeking a detail-oriented and compassionate LTSS Coordinator / Case Management Coordinator to support care management operations. This role involves coordinating non-clinical patient services, conducting member outreach, and ensuring patients receive appropriate care and resources. The ideal candidate will have experience in healthcare coordination, managed care, or case management support.
Responsibilities
- Conduct monthly outreach calls to assigned members and complete LTSS & SDOH assessments
- Coordinate non-clinical services including appointments, transportation, medication support, and DME
- Assist Case Managers with administrative and coordination tasks
- Ensure members are connected to appropriate healthcare and community resources
- Maintain accurate documentation in DCMS and other systems in compliance with HIPAA
- Identify issues and escalate clinical concerns to Care Managers
- Support patient discharge coordination including transportation and facility placements
- Collaborate with interdisciplinary teams to improve patient outcomes and reduce gaps in care
- Assist with reporting, data tracking, and quality improvement initiatives
Requirements
- Minimum 4 years of healthcare-related experience (preferred in managed care, case management, or care coordination)
- Associate’s degree required ; Bachelor’s preferred in healthcare-related field
- Strong communication (verbal & written) and interpersonal skills
- Experience with patient/member interaction and telephonic outreach
- Proficiency in EMR systems, DCMS, and Microsoft Excel
- Ability to multitask, problem-solve, and work independently
Preferred Qualifications
- Experience in managed care or health plans (Medicaid/Medicare)
- Background in care coordination, community health, or health education
- Familiarity with telephonic case management and patient engagement
