Job Description
Therapy/Rehabilitation - SLP Home Health

Schedule

Day shift, 5 days a week, from 08:00 to 16:30.

Assignment Details

6 visits per day. Guaranteed hours: 3 shifts per 13 weeks.

Requirements

• 1 year of experience required.
• BLS certification required.
• Skills in home healthcare are required.
• Must be willing to float within designated teams.
• Local candidates are accepted within a 40-80 mile radius.
• Mileage reimbursed at $0.55/mile.
• No pending licenses accepted.
• Must have no more than 7 days of RTO restrictions.

Responsibilities

• Provide speech-language pathology services in a home health setting.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress and update care plans as necessary.
• Collaborate with interdisciplinary teams to ensure optimal patient care.
• Educate patients and families on speech and language disorders.
