Registered Nurse (RN) - LTC

Position Overview

The LTC RN delivers nursing care in a long-term care facility, focusing on daily management of chronic conditions and frail older adults. The role emphasizes accurate assessment, preventive care, medication administration, and coordination with primary care providers and families to support comfort and safety.

The nurse collaborates with an interdisciplinary team to develop care plans, monitor changes in condition, and ensure smooth transitions within the facility or to higher levels of care when needed.

Responsibilities

• Conduct comprehensive assessments and monitor residents for changes in health status.
• Administer medications and manage IV therapy as prescribed.
• Perform wound care and skin integrity management, including pressure ulcer prevention.
• Identify and escalate acute changes; implement care plans in collaboration with the team.
• Coordinate with physicians, therapists, and social workers to develop and adjust individualized care plans.
• Document care, treatments, and resident responses accurately in the electronic health record.
• Educate residents and families on disease management, medications, and safety planning.

Qualifications

Education:

Associate degree in Nursing or Bachelor of Science in Nursing from an accredited program.

Licensure:

Current and valid RN license in the state of practice.

Certifications:

• Basic Life Support (BLS) certification
• Geriatric Nursing Certification (CGN) or similar gerontology credential preferred

Experience or Skills:

Experience in long-term care or geriatrics; strong communication and care coordination skills; proficiency with documentation systems.

Experience

• Experience in long-term care or skilled nursing facility settings
• Experience with chronic disease management and geriatrics

Preferred

• BSN degree preferred
• Geriatric certification or continuing education in gerontology
• Experience with electronic health records and care coordination tools

Work Environment

Care is delivered in a skilled nursing or retirement community with multi-disciplinary teams. Shifts may include days, evenings, or weekends with varying patient acuity.

Documentation is completed in the facility's electronic health record; emphasis on safety, comfort, and family communication.
