We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us. We pride ourselves on very low employee turnover. We are looking for a qualified RN Case Manager who wants a long term position with a great company. We're currently looking for a nurse to focus more on hospice but our RN Case Managers generally do both Hospice and Home Health. As a Hospice and Home Health RN Case Manager your duties will include but not be limited to:Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.Create patient centered goals with patients, family members and providersAssess, supervise and direct patient careDevelop plans of treatmentEnsure chart compliance with state regulationsCoordinate patient care with physicians and other providersHospice and Home Health are both challenging and rewarding!
