Sustainability Accompaniment Lead Role Overview & Responsibilities Position Summary The Sustainability Accompaniment Lead (Prosperemos Juntos | Thriving Together – PJTT) accompanies community-driven coalitions as they strengthen sustainability and continue shifting and sharing power through health equity strategies that address root causes of inequities. In the PJTT context, accompaniment requires a high level of discretion, independent judgment, and relational skill while guiding coalitions toward practical and realistic sustainability strategies. This role serves as a program officer, subject matter expert, and coalition coach. The Sustainability Accompaniment Lead is expected to anticipate challenges, identify sustainability gaps, and balance day-to-day execution with long-term strategic thinking. Success in this position requires comfort operating in dynamic community environments and representing MHM with confidence, humility, and strategic clarity. The annual salary for this position starts at $66,079.00, with a midpoint of $84,251.00. Most new hires begin within this range, based on their years of directly related experience and education. Candidates with exceptional qualifications that exceed the minimum requirements may be considered for compensation above the midpoint, in alignment with internal equity and IPM (Individual Pay Model) compensation guidelines. Scope and Impact The role influences community coalitions across MHM’s service area, guiding them in implementing and sustaining equitable strategies that impact health outcomes and community well-being. This position has a regional impact, requiring up to 50% travel, and contributes to MHM’s broader health equity initiatives by strengthening coalition governance, cross-sector partnerships, and community engagement. Decision-Making Authority This role operates with significant autonomy in dynamic community environments. The Sustainability Accompaniment Lead exercises sound judgment in real time while accompanying coalitions, representing MHM with professionalism, humility, and strategic clarity. The position requires discretion in navigating complex community dynamics, anticipating sustainability challenges, and making informed decisions aligned with PJTT principles and long-term coalition impact. Interactions / Working Relationships Frequent collaboration with PJTT and Communities of Solutions (CoS) team members. Regular engagement with community coalitions, Community Connectors, persons with lived experience, faith communities, schools, nonprofits, and other external stakeholders. Partnership with internal MHM departments to integrate health equity practices and advance community-driven initiatives Essential Duties and Responsibilities Manage trust-based philanthropic relationships with coalitions by developing trust, providing guidance, monitoring performance & impact, and engaging MHM colleagues. Reteach and reinforce use of PJTT frameworks (e.g., Pathways to Population Health, Vital Conditions for Community Well-Being, stakeholder mapping, driver diagrams, 90-day action planning) in public and community settings. Present on financial sustainability topics (e.g., diversified funding, business planning, grant writing, impact measurement, succession planning). Serve as a subject matter expert advising coalitions on developing and evaluating generative sustainability strategies across people, processes, resources, and impact. Contribute to the development and continuous improvement of sustainability resources such as trainings, toolkits, templates, webinars, and consultant support. Guide coalitions in governance, strategic planning, action planning, and financial transparency to promote shifting and sharing of power. Participate in the planning, implementation, and continuous improvement of PJTT events. Co-design and co-evaluate PJTT efforts with other PJTT team members, Community Connectors, and persons with lived experience. Collaborate across MHM to embed equity principles and resident voice into planning and decision-making. Facilitate community discussions to share information, collect insights, and support program evaluation. Build and maintain strong relationships with external partners including nonprofits, schools, faith-based institutions, and community stakeholders. Compile and analyze data to support evaluation and reporting on coalition progress toward power-shifting, health equity, and sustainability goals. Participate in curriculum development and tactical planning for PJTT. Represent MHM at community meetings and task forces. Perform other duties as assigned. Approximate % of role: Coalition coaching/support 40%; Presentations & facilitation 20%; Capacity-building 25%; Reporting/other duties 15%. Supervisory or Leadership Responsibilities This position does not supervise others but provides coaching, facilitation, and subject matter expertise to coalitions, Community Connectors, and cross-sector partners. Qualifications Minimum Qualifications (Required) Education: Bachelor’s degree in social/behavioral sciences, public health, or a related field. Three (3) years of experience in community outreach, organizing, or coalition building, including direct work with individuals who have lived experiences of inequities. In lieu of a degree: High school diploma or equivalent and five (5) years of progressively responsible experience in community outreach, organizing, or coalition-building roles. Licenses/Certifications: None Experience: Demonstrated experience engaging individuals with lived experience in projects, programs, and/or decision-making processes to shift power to community members. Experience and knowledge in conflict resolution. Preferred Qualifications Experience sustaining people, processes, resources, and impact in nonprofit or public-private partnerships Familiarity with sustainability strategies (diversified funding, business planning, impact measurement) Knowledge of community development models such as ABCD Experience with health/social inequities work in marginalized communities Experience with grassroots organizing and faith-based partnerships Strong project management, public speaking, facilitation, and relationship-building expertise Familiarity with grant management systems or reporting Knowledge, Skills, and Abilities (KSAs) Collaboration, communication, conflict resolution, and problem-solving. Strategic and analytical thinking with a learning-oriented, “fail forward” mindset. Strong facilitation skills to engage diverse community voices. Relationship-building across sectors with cultural competence and adaptability. Language Skills Required: Fluency in English (written and spoken). Preferred: Bilingual in Spanish, with ability to present and respond to diverse audiences, develop documents, and facilitate bilingual discussions. Team members who demonstrate fluency in both Spanish and English will receive additional pay and be expected to translate small amounts of written text from English to Spanish, especially when working under time constraints (not to exceed 5% of the team member’s total weekly time) Technology and Tools Microsoft Office Suite (Outlook, Word, Excel, PowerPoint). Virtual collaboration tools (Zoom, MS Teams). Proficient navigating online platforms and data reporting systems. Work Environment and Physical Demands Hybrid role with office, community, and virtual settings. Travel required up to 50% within assigned regions and to San Antonio. Evening or weekend participation in community events may occur. Physical requirements include bending, standing, walking, and lifting up to 25 pounds.
