Clinical Pharmacist — Primary Care & Medication Optimization

DotHouse Health in Boston is seeking a Clinical Pharmacist to join our integrated primary care team. The Clinical Pharmacist serves as a clinical resource for patients, providers and staff and as a strategic thought partner across the health center. Working collaboratively with primary care providers, nurses, population health staff, and the on-site pharmacy. The Clinical Pharmacist helps optimize medication therapy, improve chronic disease outcomes, and remove barriers to medication access. #J-18808-Ljbffr DotHouse Health
