Shift: 6:45am - 7:15pm or 6:45pm - 7:15am (Varies)

Job Description:

Patient Population: The patient population served in the DP/SNF is 21 years old and older. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

Position Summary: Under the direct supervision of a licensed nurse, the Certified Nursing Assistant (C.N.A.) performs individually assigned patient care activities based upon established patient care policies and procedures. Specific areas of responsibility include: monitoring of patient vital signs, providing patient hygiene and comfort measures, assisting with medical procedures, fulfilling nutritional needs, and maintenance of a record of the care provided as assigned. Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act. (Gov't. Code 3100, 3102) Needs to recognize that they have an affirmative duty and responsibility for reporting perceived misconduct, including actual or potential violations of laws, regulations, policies, procedures, or this organization's standards/code of conduct. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have basic computer skills.

Licensure/Certifications: To perform this job successfully, an individual must have and maintain current licensure as a Certified Nursing Assistant (C.N.A.) in the state of California. Must have and maintain BLS certification or obtain within orientation process, prior to hands-on patient care.

Responsibilities and Essential Functions:

• Demonstrates support of the philosophy, policies and goals of Patient Care Services and the individual unit.
• Documents care on the patient care record, accurately, consistently, and in a timely manner.
• Prioritizes work activities to achieve optimal patient care.
• Demonstrates ability to identify and anticipate patient care needs.
• Is responsible for the treatment of care appropriate to the ages of the patients served, i.e. Adult (21 years of age) and Geriatric.
• Delivers basic nursing care, utilizing policies, procedures and standards of care.
• Practices appropriate infection control techniques.
• Responsible for general clean-up and tidiness of patient rooms, kitchen, refrigerator (check daily, clean weekly and prn), lounge, and countertops.
• Demonstrates adherence to the "National Patient Safety Goals for Hospitals" as defined by scope of practice.
• Attends unit, hospital and other available in-service programs according to education policy.
