As a Nurse Care Manager, you’ll be at the heart of client wellness, supporting individuals with chronic conditions and behavioral health needs. What You’ll Do Collaborate with an interdisciplinary team to develop client-centered wellness plans Conduct health and metabolic screenings (quarterly or annually) Provide outreach calls to engage and support clients Lead health and wellness education sessions and group activities Monitor hospital admissions/discharges and coordinate medication reconciliation Partner with healthcare providers to ensure continuity of care Track and assess performance measures related to physical health Occasionally (up to 25%) meet clients in the community or in their homes Requirements of the Position Graduation from an accredited nursing program Active, unrestricted Missouri LPN or RN license Valid driver’s license, proof of insurance, and a reliable vehicle Pediatric experience or experience working with children 5 years old and up preferred.
