RN Case Manager-ALP Located in Brooklyn, NY *Salary: $100K to $105K per year (dependind on experience)* Responsibilites: Will take responsibility for resident care, supervision of staff, working closely with MD and picking up MD orders, writing case notes and ensuring all residents have their 6 and 12 month medical evaluations. Act as the on-site point of contact for operational issues when administration is unavailable. Qualifications: Must have a valid New York RN license UAS certification and experience completing UAS assessments strongly preferred Must have prior experience as a Case Manager Must have experience in a geriatric setting, preferably an assisted living program Strong written and verbal communication skills Offering an excellent benefit package including health insurance with HRA (NCHHC covers the deductible), dental, vision, company paid life insurance, 403b plan with company match and company annual contribution after 1 year of service.
