SDOH Navigator — Patient Resource & Benefits Connector

Erie Family is seeking an SDOH Navigator in Evanston to support patients by phone and with the care team across sites. The role screens for social drivers of health needs and links patients to Erie resources and community programs. You will enroll patients in benefits, schedule medical appointments, and assist with digital navigation such as MyChart and telehealth setup.
