Community Resource Navigator

Guide Healthcare

Guidehealth is seeking a Community Resource Guide to assist patients remotely and in person with navigating medical, behavioral health, substance use, and social systems. You will build trusting relationships, assess needs, and connect patients to resources to close gaps in care. In this role, you will support quality measures, risk adjustment, and value-based care initiatives while documenting all interactions and outcomes in EHR systems. #J-18808-Ljbffr Guide Healthcare
