Guidehealth seeks a Community Resource Guide to bridge patients, primary care physicians, specialty providers, and community resources, advancing care navigation and whole-person health. You will support patients remotely and, at times, in person to navigate health systems and access resources. This role helps reduce gaps in care and improve engagement through proactive outreach. You will collaborate with care teams, document assessments and interventions, and contribute to quality measures #J-18808-Ljbffr Guide-Health
