Discharge Care Manager RN Care Transitions Lead, Bridgeport

UnitedHealth Group

UnitedHealth Group is hiring a Discharge Care Manager in Bridgeport, CT to lead telephonic case management for high-risk members, assess needs, and coordinate solutions that promote healthier lives. You will evaluate discharge needs, coordinate post-discharge care with providers and members, and participate in rounds with the Medical Director to discuss cases.
