Weekend / Field RN / Coordinator Of CareSalary Range: $95,000 – $110,000 per year Sign-On Bonus: $20,000 Location: Westchester, NY Schedule: Friday through Tuesday, business hours Employment Type: Full-Time, Permanent Position Summary RCM Healthcare Services is seeking an experienced Registered Nurse (RN) / Coordinator of Care for a full-time, permanent opportunity with a large, respected Certified Home Health Agency (CHHA) serving Westchester County. The RN Coordinator of Care manages all aspects of assigned patients' home care services, including home visits, assessments, care planning, clinical coordination, and documentation. This position works closely with patients, families, physicians, home health aides, and interdisciplinary care teams to ensure high-quality, coordinated care in the home setting. At RCM Healthcare Services, we're passionate about connecting talented professionals with the right opportunities — join our network and be supported every step of the way.Responsibilities Schedule / ShiftFull-time, permanent positionFriday through TuesdayBusiness hoursField-based position serving patients throughout Westchester CountyKey DutiesPerform in-home nursing assessments to determine patient needs, eligibility, and appropriate plans of care.Develop, implement, monitor, and update individualized care plans based on patient needs and progress.Provide skilled nursing care, including wound care, medication management, disease education, and other clinical services.Coordinate care and communicate with patients, families, physicians, home health aides, and interdisciplinary team members.Complete accurate and timely clinical documentation using HCHB or other electronic medical record systems.Identify the need for additional services, including PT, OT, speech therapy, and social work, and coordinate referrals as appropriate.Supervise and provide guidance to home health aide personnel as needed.Participate in case conferences and ensure compliance with agency policies, infection control standards, and applicable regulatory requirements.Requirements EducationGraduate of an accredited nursing program.Licenses & CertificationsCurrent and unrestricted New York State Registered Nurse (RN) license.Current physical examination as required for employment.ExperienceMinimum of 6 months of clinical/hospital nursing experience.Home care or CHHA experience is preferred.HCHB experience is a plus.Vehicle required for most territories.SkillsStrong clinical assessment and care coordination skills.Excellent communication, organization, and documentation abilities.Ability to work independently in a field-based home care environment.Comfortable using electronic medical record systems; familiarity with HCHB is preferred.Ability to collaborate effectively with patients, families, physicians, and interdisciplinary care teams.Benefits$20,000 sign-on bonusUp to $40,000 in tuition reimbursement, with up to $10,000 available annually.Excellent health benefits.Four weeks of vacation.Eight sick days plus additional paid holidays.Stable, supportive work environment with opportunities for professional growth.About Us RCM Healthcare Services works with professionals across schools, healthcare, and community-based settings nationwide.
