Brighton Home Health and Hospice - OR
Brighton/Suncrest Hospice is seeking a Hospice Care Consultant to strengthen referrals and partnerships across our service area. You will interact with physicians, hospitals, and community partners to ensure continued delivery of high-quality hospice care. You should be a self-starter capable of working with an interdisciplinary team of hospice professionals, building relationships with referral sources, payers, and patients to support our mission. #J-18808-Ljbffr
