Your Responsibilities

• Conduct thorough patient evaluations, recommend diagnostic tests, and create personalized care strategies
• Diagnose and manage acute, episodic, or chronic conditions, either independently or in collaboration with physicians (as per your credentialing)
• Engage with patients and their families to educate them on prevention, wellness, and long-term care management
• Administer prescriptions and therapies aligned with your practice scope
• Play an active role in patient follow-ups and chronic care management initiatives
• Work collaboratively with multidisciplinary teams to provide cohesive, patient-centered care
• Maintain accurate and comprehensive records in electronic medical records (EMR)

Qualifications

• Master of Science in Nursing (MSN) or a Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Licensing and Certifications

• Active Nurse Practitioner license in the state of Kentucky
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or equivalent certification in your specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the willingness to obtain these within 90 days)
