Pay & Benefits

Annual Salary Range: $80,000 – $115,000 (dependent on experience)

Pay Package: Discussed upon application

What You'll Do

• Assess and develop treatment plans for outpatient and inpatient populations
• Provide therapy services across multiple settings, including occasional SNF and home health visits
• Document patient progress using Epic EMR accurately and timely
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Participate in weekend and holiday rotation as scheduled
• Promote patient education and engagement in therapy programs

What You Need

• Minnesota Physical Therapist license
• BLS certification (within 1 year of hire)
• At least 1 year of relevant healthcare experience
• Proficiency with electronic medical records (preferably Epic)

• Experience in outpatient and inpatient rehab settings
• Strong communication and team collaboration skills
