Social Work Case Manager

Social Work Case Manager Location: Virtual full-time, with required in‑person training sessions and occasional office visits. Work schedule: Monday-Friday, 10:30 AM – 7:00 PM EST. Responsibilities How You Will Make An Impact Utilize community, government, and client resources to address participants’ limitations and support interventions in the management of chronic conditions. Manage behavioral and psychosocial needs that improve clinical and financial outcomes and deliver social work interventions. Assist members in effectively using resources to meet personal health needs and developing their own capabilities. Evaluate members’ ability to self‑manage and locate alternative resources when limitations are identified via a psychosocial evaluation. Provide guidance to members seeking alternative solutions to social, cultural, or financial problems affecting healthcare management. Evaluate members’ strengths related to health self‑management, develop strategies to support needs, and implement plans for case decisions. Facilitate and coordinate behavioral health resources as individual member needs arise. Requirements MS (at a minimum) in Social Work and a minimum of 3 years of case management experience in a health‑care environment, or equivalent education and experience. Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s). Preferred Qualifications Bilingual (Spanish) or other multi‑language skills.
