Long Term Care Lpn (Temp To Perm)

Key Responsibilities:

• Provide high-quality patient care in a long-term care setting, adhering to all state and facility regulations.
• Assess patient needs, develop care plans, and implement interventions to promote well-being.
• Collaborate with interdisciplinary teams to ensure seamless patient care and communication.
• Maintain accurate and detailed documentation of patient care activities and progress.
• Ensure compliance with safety standards, infection control policies, and facility protocols.

Qualifications:

• Valid LPN license from the state of Iowa
• BLS certification
• Mandatory Reporter training and certification
• Minimum 2 years of experience in long-term care or skilled nursing facilities
• Flexibility with schedules
• Strong communication skills
• Ability to work rotating weekends
• Physical ability to stand for extended periods
