Housing Case Manager for Young Families & Housing Stability
$23 - $26 per hourHousing Works
Compensation Range: $23/hr - $26/hr commensurate with experience Benefits: Three comprehensive healthcare plans available. Housing Works covers most of the plan; employees pay a portion based on salary. Staff accrues PTO immediately up to 30 days in the first year. Overview The Housing Case Manager (HCM) provides case management and supportive care coordination for assigned clients and their households in the STARS 6 Young Adult family program, a scattered site supportive housing program for young families. The HCM works closely with clients in their homes, in the community, and in the office to promote housing stability, independent living skills, financial management, and health and wellness. Responsibilities Effectively manage assigned caseload, including engagement, referrals, concrete needs, and chart compliance. Provide referrals and follow‑ups for primary care, substance use, nutrition, clothing, mental health, financial management, and dental. Identify and schedule case conferences with internal and external providers. Coordinate services with internal departments (ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, Job‑Training). Maintain chart compliance in eICare and TMS (Maven Mental Health System). Assist clients in obtaining and maintaining benefits (SNAP, SSI, Medicaid, rental subsidies). Participate in interdisciplinary teams and document service provision in eICare per contract deliverables. Help new clients move into apartments and transition from homelessness to stable housing. Develop individualized, strength‑based goals with clients and provide feedback during scheduled engagement. Collaborate with Child Development Specialist to ensure services for children. Discuss financial management, including rent obligations, subsidy maintenance, utility payments, and nutritional needs. Accompany clients on scheduled appointments and travel via mass transit as needed. Participate in bi‑weekly supervision meetings and annual staff training (Motivational Interviewing I & II, Smoking Cessation, Trauma‑Informed Care). Provide crisis intervention, mediation, and coverage for other HCMs when needed. Complete reports and paperwork before deadlines. Demonstrate outstanding customer service with residents, staff, vendors, and stakeholders. Advocate and participate in actions to address homelessness and HIV/AIDS. Requirements Possess a BSW or related degree with adherence to ethical standards of the social work profession. MSW preferred. Experience with young adults in low‑income, marginalized communities and with young children. Strong communication skills, verbal and written. Proficient in Microsoft Outlook, Word, Excel, and PowerPoint. Cultural competency, especially with LGBTQI+ community. Dependable, flexible, maintain confidentiality. Excellent internal and external customer service and interpersonal skills. Ability to work under pressure and resolve problems with colleagues.
