Our dedicated team of healthcare professionals is committed to delivering personalized, high-quality support tailored to meet the unique needs of each individual we serve What We’re Searching For Hometown Home Health is seeking a dynamic individual to join our team in a Home Health Registered Nurse role. This position plays a pivotal role in maintaining top tier patient care. What You’ll Do Coordinate patient care in accordance with the treatment plan created by the physician. Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient. Participate in the patient's discharge planning process. Prepare progress notes following visits for patients’ charts. What You Need Must have a nursing degree from an accredited program Licensed as a registered nurse within the state of Florida Adherence to federal, state, and accreditation requirements, including Medicare and Medicaid regulations. CPR certification required (Online certification not accepted). At least one year of experience at a home health agency or within a hospital. Who is RallyX?
