Patient Care Manager

The Home Health Patient Care Manager oversees clinical services and coordinates an interdisciplinary team to ensure high-quality, individualized care for home health patients in compliance with all regulations. Responsibilities include providing clinical care within the scope of practice, managing patient referrals and assessments, evaluating insurance and benefit approvals, and reviewing care plans daily. The role requires the ability to adapt to diverse home situations and deliver personalized care based on physician orders. Qualifications include current unrestricted RN licensure and reliable transportation. Preferred skills are CPR certification and home care experience.
