MSW Social Worker

Job Summary:
We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to provide skilled nursing care, case management, and hospice care. The role involves patient assessments, medication administration, treatment planning, and interdisciplinary collaboration, with a focus on discharge planning. The candidate must have at least 2 years of acute hospital case management experience and be able to work weekends and holidays as required.
Responsibilities:
We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to provide skilled nursing care, case management, and hospice care. The role involves patient assessments, medication administration, treatment planning, and interdisciplinary collaboration, with a focus on discharge planning. The candidate must have at least 2 years of acute hospital case management experience and be able to work weekends and holidays as required.
Responsibilities:
- Provide skilled nursing care including patient assessments and medication administration.
- Develop and implement treatment plans in collaboration with the interdisciplinary team.
- Manage case load with a ratio of approximately 1:35 patients.
- Focus on discharge planning to ensure smooth patient transitions.
- Cover up to 2 units on weekends as scheduled.
- Participate in holiday coverage as required.
- Minimum 2 years of acute hospital case management experience.
- Active Registered Nurse license in the state of practice.
- Clinical assessment, medication administration, and treatment planning skills.
- Interdisciplinary team collaboration skills.
- Excellent communication and interpersonal skills.
- Bachelor's degree in Nursing or related field.
- Driver's license, car insurance, and automobile for field work (if applicable).
- Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
- Experience with discharge planning and case management in acute care.
- Weekend coverage required every other weekend.
- Must be available for holiday coverage.
- Return to Office (RTO) restrictions limit time in office to no more than 7 days.
- Work environment is an acute care facility.
- Scrub or business casual attire required.
- Work schedule includes weekend shifts every other weekend.
- Shift assignments may include coverage of up to 2 units on weekends.
- Holiday shifts as assigned.
