Behavioral Health Care Advocate

Responsibilities Conduct comprehensive behavioral health screenings in a dynamic environment that may include local county jails. Identify and maintain awareness and contacts of specific community resources for justice‑involved populations and manage enrollment processes. Identify and coordinate appropriate levels of care linkage to community behavioral health and ancillary services. Assess safety risks and provide crisis management interventions. Provide short‑term clinical case management to ensure continuity of behavioral health and medical services. Review available behavioral health history to coordinate services and assure appropriate level of care for clients. Navigate and advocate within the behavioral health system of care on behalf of justice‑involved clients as needed. Engage clients in the screening process and collaborate with a multi‑disciplinary team to develop case plans, providing linkage to care based on individual needs, preferences, and objectives. Utilize motivational interviewing techniques to help clients identify and understand intrinsic goals and engage in behavior change. Participate in ongoing multi‑disciplinary team meetings, providing education and consultation regarding linkage to behavioral health (BEH041 – BHST 4/2019) and ancillary services within the system of care. Provide clinical care consultations with California Department of Corrections and Rehabilitation personnel, probation officers, and community treatment providers. Maintain timely and appropriate clinical documentation records in San Diego County Probation and Behavioral Health Electronic Health records. Use technology systems to support clinical delivery of services. Operate within Cerner Electronic Health Record, handling Protected Health Information and the Probation Case Management System. Adhere to pertinent HIPAA rules and regulations. Perform other duties and responsibilities as required. Job Details Location: Community Transition Center in San Diego, with minimal travel (no more than 25%) to various probation offices and jails within the SD area. Schedule: Monday – Friday, 8:00 am – 4:30 pm. Sign‑on Bonus: $5,000 for external candidates. Required Qualifications Registered psychiatric nurse or California‑licensed master’s‑level mental health professional. 3+ years providing direct clinical care in a behavioral health treatment setting with emphasis on assessment, case management, and crisis intervention; community behavioral health experience required.
