Therapist - Physical Therapist

Job Summary:

Provide outpatient physical therapy services to home health patients, focusing on individualized care plans and patient progress. The therapist will conduct home visits, document treatment outcomes, and collaborate with the care team to support patient recovery.

Responsibilities:

• 
  Assess patients' physical condition and develop personalized treatment plans based on clinical evaluations.
  
  
• 
  Conduct home visits to deliver therapy services, ensuring patient safety and comfort during each session.
  
  
• 
  Document patient progress, treatment details, and relevant observations accurately within established time frames.
  
  
• 
  Collaborate with physicians and care team members to modify therapy plans, ensuring optimal patient outcomes.
  
  
• 
  Maintain compliance with all regulatory requirements, including CPR, BLS, and agency-specific protocols.
  
  
• 
  Travel up to 75 miles in the assigned territory to reach patients in need of therapy services.
  
  

Qualifications:

• 
  Hold a valid state license to practice as a Physical Therapist; license must be in hand prior to start.
  
  
• 
  Possess a minimum of 1 year of recent adult home health or hospice therapy experience.
  
  
• 
  Demonstrate proficiency with EMR systems such as HomeCare HomeBase (HCHB).
  
  
• 
  Be proficient in OASIS documentation (if applicable), with recent use or completed retraining.
  
  
• 
  Have reliable transportation, a valid drivers license, and current auto insurance.
  
  
• 
  Be willing to travel and accept home visits across designated territory.
