• Direct Patient Care: Coordinates and delivers restorative physical therapy at the center and in participants' homes to restore function, safety, and independence.
• Interdisciplinary Collaboration: Works alongside PACE physicians, Occupational Therapists, and rehabilitation staff to run the department efficiently.
• Leadership & Culture: Acts as a member of the management team, fostering internal and external relationships that align with WelbeHealth's mission and values.

Duties and Responsibilities:
-Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
-Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
-Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).
-Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
-Order, track and manage maintenance for equipment.
-Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or functional status.
-Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)
-In coordination with the Marketing Team, support enrollment of prospective participants into the program.
-Participate in end-of-life care coordination and support.
-Manage direct reports including training, supervising and mentoring.
-Be legally authorized to practice in position location, required.
-Only act within the scope of his or her authority to practice.
-Complete all required documentation in a timely and accurate manner.
-Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
-Follow WelbeHealth policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
-Communicate regularly with Supervisor and team regarding workload and priorities.
-Timely completion of all mandated trainings and education.
-Timely completion of all mandated occupational health screenings as needed.
-Exercises flexibility in performing assignments as business needs evolve.
-Other duties as assigned

Skills:

Required Skills & Experience:
-Five years of Progressively responsible PT experience.
-Minimum of two years’ Physical Therapy experience in a clinical setting with a frail or elderly population.
-Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
-Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
-Two years’ supervisory experience with demonstrated ability to mentor and develop team members
-Ability to process accurate information in a timely manner.
-Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members
-Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
-Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.
-Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
-Ability to work independently with minimal supervision
-Demonstrated ability to prioritize in a fast-paced environment.
