Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, hospital policies, and professional guidelines. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among staff and other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include conducting nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. Key responsibilities include comprehensive patient assessments. Consideration of the patient’s culture, age, and developmental level is essential. The RN identifies patient risk factors and presenting complaints. Collecting objective and subjective data is part of the assessment process. The RN develops a prioritized problem list based on input from the inter-professional team. Actions and interventions are designed considering the patient’s culture and age. The RN determines desired outcomes in coordination with the patient and family. Patient care therapies and procedures are implemented within established policies and the RN's scope of practice. The RN explains procedures, treatments, and processes to patients and families. Initiating action to alleviate patient stress and discomfort is crucial. Recognizing, responding to, and communicating changes in patient conditions appropriately is required. Ongoing assessment and documentation in response to procedures and therapies are conducted according to hospital policy. The RN delegates tasks appropriately and oversees team members performing patient care activities. Monitoring the accuracy and completeness of delegated care is essential. Identifying and addressing patients' educational needs related to prioritized problems is part of the role. The RN facilitates the discharge process by executing discharge orders promptly. Providing input into patient/nurse assignments considering available staffing and patient acuity is necessary. Active participation in unit-based activities supports organizational values and priorities. The RN promotes a positive group dynamic and healing environment. Assuring nursing excellence according to accepted standards and evidence-based practice is fundamental. Participation in department and unit quality improvement activities is expected. A holistic perspective regarding patient care is essential to success in this role.
