Registered Nurse (RN) - Case Management

Registered Nurse Case Manager

Schedule: Day 5x8-Hour (08:00 - 16:30)

Assignment Details: Contract length: Not specified. No block schedules.

Requirements

• Must have a valid CA state license (pending license accepted, must be licensed upon start date)
• 2 years of Acute Care Case Management/Discharge Planning experience required; Behavioral health experience does not apply
• No more than 7 days of requested time off
• Availability to work every other weekend

Responsibilities

• Manage patient care and discharge planning processes
• Coordinate with interdisciplinary teams to ensure effective case management
• Facilitate communication between patients, families, and healthcare providers
• Monitor progress and adjust discharge plans as necessary
