Under general supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures. Focuses on the prevention of illness, injury or disability, the promotion of health and maintenance of the health of populations. Cares for population with special needs during disasters/emergencies and biological and chemical protocols.

Typical tasks include: identifying high risk populations and tailoring interventions based on the needs of the population and the resources available; providing treatments, administering medications/vaccines and providing preventive and maintenance care to patient/families consistent with nurse protocols; screening and referring clients to other community service providers; participating in the planning, organizing and evaluation of program areas to ensure the goals and objective(s) of the program are met; providing clinical services to target population; applying principles of population health to practice; conducting comprehensive assessments of client's health care needs; developing comprehensive plans of care for individual, families, and populations; modifying client treatment plans as indicated by client's responses and conditions; providing health education instruction and/or classes to individuals, families, for the promotion and maintenance of health; developing and sustaining community partnerships; participating in drills and exercises on pandemic influenza, mass sheltering, and mass dispensing; performing job responsibilities with minimal supervision; managing specific program activities and/or specific program evaluations; cross training in various clinical services/programs; and providing additional duties as assigned.

Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND one year of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
