Registered Nurse Home Care

Job Details

Provides, coordinates, teaches, and supervises nursing care for a group of patients in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and caregivers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients identified needs and priorities for care. Complies with all Home Health Services policies and procedures.

Work Location: Baltimore County, Baltimore City, Harford County, Howard County, and/or Anne Arundel County (you will be assigned to specific territory)

Currently only hiring into the following territories: Baltimore County, Baltimore City, Harford County, Howard County

Work Hours: must be available from 8:30a-5p. However, patient visit times are flexible.

To succeed in this role, you will need the following:

• BSN required
• Current RN licensure
• Active American Heart Association or Red Cross BLS CPR
• Valid Maryland drivers' license and current auto insurance, working vehicle;
• At least 2 years of clinical nursing practice required in Acute Care, ICU or ED
• 2 years of home care experience preferred
• Passion for educating patients in their home environment a MUST.
• Experience with electronic documentation (i.e. Epic).
