Requirements

• Active Vermont RN License (Required)
• Minimum Home Health experience preferred
• OASIS experience required
• Current BLS/CPR Certification
• Acute Care nursing experience preferred
• Experience with Homecare Homebase ( HCHB ) preferred
• Valid driver's license with reliable, insured vehicle
• Strong assessment, documentation, communication, and case management skills
