You'll be responsible for:

• Performing routine and follow-up medical evaluations
• Managing chronic and acute medical conditions
• Collaborating with physicians and interdisciplinary care teams
• Developing individualized treatment plans
• Reviewing labs, diagnostics, and medications
• Communicating with patients, families, and nursing staff
• Helping to prevent unnecessary hospitalizations
• Focusing on delivering compassionate, patient-centered care
• Completing documentation with the support of our medical scribes

What you'll bring to the table:

Here are some qualifications that we need to join our team...

• Active New York State Nurse Practitioner License.
• DEA Registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Experience in: Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long-Term Care, Sub-Acute Care.
