Ideal Candidate:
- MD or DO degree
- Active U.S. medical license
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or Palliative Care
- Passion for adult primary care and chronic disease management
- Interest or experience in value-based care
- Excellent communication and interpersonal skills
