Registered Nurse (RN) - Case Manager
We are seeking a dedicated and experienced Registered Nurse (RN) for a Case Manager position. The ideal candidate will have recent acute care discharge planning experience and will be responsible for coordinating patient care and ensuring smooth transitions from hospital to home or other care facilities.
General Information
- Unit Specifics:
- Number of Beds: 590
- Patient Ratios: 1 RN to 25 patients, varies
- Unit Staffing Description: RN, SW, and CMA
- Documentation System: Meditech, Midas, Wellsky
- Scheduling Requirements: Monday to Friday from 8:00 AM to 4:30 PM with rotating weekends
- Floating Policy: May float to any unit as needed
Qualifications
- Must possess recent acute care discharge planning experience.
- Strong communication and organizational skills.
- Ability to work collaboratively in a team environment.
- Ability to adapt to changing patient care needs and priorities.
Dress Code
Business casual attire is required.
