Description

• Conduct assessments of patients in either inpatient or outpatient settings to create personalized treatment plans

• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home exercise regimens
• Accurately document evaluations, treatments, and patient progress in compliance with facility and regulatory standards
• Engage in team rounds and interdisciplinary conferences to promote coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
