DescriptionJob TitleStroke CoordinatorDepartmentQuality AssuranceThis position is exempt from Career Service under the CCH Personnel Rules.Job SummaryThe Stroke Coordinator works under the direction of Quality Assurance and in collaboration with the Chair of the Division of Neurology-Clinical in managing, planning, development, and overseeing the Stroke Program for Cook County Health (CCH). The Stroke Coordinator has excellent organizational skills, is attentive to detail, and works well with patients, nurses, physicians, and hospital staff. The Stroke Coordinator is responsible for ensuring Stroke Program compliance with Joint Commission requirements for Primary Stroke Center Certification and works with others to ensure compliance with Joint Commission Primary Stroke Center Performance Measures and other Joint Commission requirements. Represents the Stroke Program in Leadership conferences, collecting, analyzing and presenting daily stroke data, and partnering with Nursing to ensure training and competency of all front-line nurses is completed annually. The Stroke Coordinator develops Stroke Program policies, protocols, guidelines, clinical pathways, and care sets.Typical Duties· Assists in development of all Program policies, protocols, guidelines, pathways and care sets; monitors for compliance; maintains currency and accuracy· Works closely with Quality Assurance to maintain Performance Measure data collection, report generation and analysis as well as accurate submissions to the Joint Commission· Maintains a stroke registry in real-time to include patient diagnoses, performance measures, treatments and outcomes and timeliness of stroke team, radiology, lab responses· Remains current of and compliance with all Joint Commission Primary, Thrombectomy-Capable and Comprehensive Stroke Center requirements· Responsible for collection, analysis, accuracy and reporting of stroke patient data through maintenance of a computerized stroke registry; develops benchmarks and comparisons to national standards· Develops and initiates action plans necessary to meet performance standards· Meets with each hospitalized stroke patient for purposes of stroke education and accuracy of stroke registry data collection· Responsible for the development, organization, and maintenance for all the Program’s professional educational activities and training of staff for system-wide stroke education, including annual nursing dysphagia screening and neurologic check competencies· Maintains records of all system stroke-related educational activities; maintains records of all stroke-related activities of Stroke Team members; serves as an educational resource and leader for the hospital’s nursing and medical staff on all stroke related quality issues· Maintains schedule of Stroke Team Assignments; maintains organizational chart, list of personnel, list of resources· Participates in outpatient clinics providing stroke-related care· Fosters and builds collaborative relationships with internal and external stakeholders to facilitate a productive, collaborative, multidisciplinary working environment in which excellence of stroke care is the objective; coordinates stroke-related activities of all hospital departments· Engages with staff throughout CCH clinics to facilitate care of stroke patients, optimize resource utilization, and supports system-wide training coordination to ensure Stroke readiness· Engages and participates in stroke-related research efforts· Participates in local, regional, national and international stroke associations, agencies and groups; attends local and national stroke-related meetings· Maintains constructive working relationships with membership of local hospital stroke programs· Serves as resource for and liaison to community organizations interested in stroke· Partners with interdepartmental nursing leadership to ensure ongoing stroke competency and compliance CCH system wide· Performs other duties as assignedQualificationsMinimum Qualifications· Licensed as a Registered Professional Nurse in the State of Illinois· Bachelor’s degree or higher in Nursing from an accredited college or university· Four (4) years of nursing experience· One (1) year of Neurosurgical or Stroke Nurse experience· Current Quality Improvement experience· Current Basic Life Support (BLS) certification from the American Heart Association· Proficient in Microsoft Office (Excel, Power Point, and Word)Preferred Qualifications· Master’s degree in nursing from an accredited college or university· Current licensure as an Advanced Practice Nurse – Clinical Nurse Specialist in the State of Illinois· Certification in one of the following: American Association of Critical Care Nurses as a CNS (CCNS), Board of Certification of Emergency Nurses (BCEN), or American Association of Neuroscience Nurses (AANN)· Previous Stroke program coordination experience· Previous clinical experience in training and education of staff· Previous experience in patient education· Previous experience in Regulatory Compliance survey participation· Current experience maintaining an electronic registry, preferably a stroke registryKnowledge, Skills, Abilities and Other Characteristics· Knowledge of state and federal laws, rules, and regulations related to the care of patients with stroke· Knowledge of Joint Commission requirements for Primary Stroke Center certification· Knowledge and is experienced in the nursing care of patients with stroke· Working knowledge of electronic medical record database· Working knowledge of Microsoft Office (Word, Excel, PowerPoint, Access)· Excellent interpersonal skills to establish and maintain appropriate professional relationships with patients, members of the health care team, and other personnel· Excellent oral, verbal and written communication skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups· Sufficient organizational skills to manage diverse group of complex patients· Ability to assess information, apply critical thinking to determine course of action, and relay this information to patients, families, and clinical staff· Ability to demonstrate respect and sensitivity for cultural diversity, patients and coworkers of all sexual orientations and gender identities· Ability to effectively communicate while maintaining Health Insurance Portability and Accountability Act (HIPPA) for Protected Health Information (PHI)· Strong written and verbal communication· Ability to function autonomously and as part of a multidisciplinary team· Ability to see and hear clearly (including with correction)· Ability to tolerate exposure to unpleasant noise, odor and temperatureBenefits PackageMedical, Dental, and Vision CoverageBasic Term Life InsurancePension PlanDeferred Compensation ProgramPaid Holidays, Vacation, and Sick TimeYou may also qualify for the Public Service Loan Forgiveness Program (PSLF) 100% Tuition Reimbursement for nursing-related programs For further information on our excellent benefits package, please click on the following ink: Physical and Environmental DemandsThis position is functioning within a healthcare environment.
