Registered Nurse (RN) Case Manager opportunity in Arlington, VA - 13-week travel contract coordinating acute inpatient care, discharge planning, and safe patient transitions.

• Location: On-site in Arlington, VA (22205) - Northern Virginia / Washington, D.C. area
• Shifts per Week: 5
• Shift Duration: 8 Hour Days (8:30am - 5:00pm)

Qualifications

• Associate Nursing Degree required
• Minimum 1 year acute medical/surgical, emergency department, or critical care nursing experience
• Experience using electronic medical record (EMR) systems
• Knowledge of healthcare reimbursement systems (Medicare Advantage, Commercial payer, Medicaid) and CMS regulatory rules
• Completion of Patient Care Coordinator Assessment of Critical Skills
• Strong communication, care coordination, and utilization review skills

Responsibilities

• Oversee care coordination and case management for the acute inpatient population as the RN Case Manager
• Collaborate with physicians, nursing, utilization review, ambulatory case managers, and patients/families to ensure continuum of care and clinical/cost-effective outcomes
• Facilitate transitions of care and coordinate with community providers and in-network vendors (Home Health, DME, IV Infusion, SNF Sub-Acute, Acute Rehab)
• Complete initial face-to-face assessment for every admitted member within 24 hours of admission to identify discharge needs and determine transition plans
• Document and communicate assessment outcomes and discharge plans in the EMR in accordance with facility and regulatory requirements
• Send referrals and coordinate post-acute care resources to support safe, patient-centered discharge planning
