Director of Case Management

We are seeking a Director of Case Management to join our team!

The Director of Case Management plays a pivotal role in ensuring the delivery of high-quality, patient-centered care while optimizing resource utilization and operational efficiency within the hospital setting. This position requires strong leadership skills, clinical expertise, and a commitment to continuous improvement in healthcare delivery.

Qualifications:

  • A bachelor’s degree in related field
  • Previous management experience over care management
  • Current licensure or certification as required by state regulations (e.g., Registered Nurse (RN), Licensed Clinical Social Worker (LCSW) preferred)
