Job Summary
Diligent Home Health Care is hiring a Home Health Scheduler to coordinate caregiver visits, ensure timely admissions, and support seamless inhome care across the greater Jacksonville service area. You will match caregivers to client needs, manage daily schedules in the EHR, communicate with clinicians and families, and quickly resolve staffing gaps. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.

Responsibilities

• Professional supervise quality, reliable, in-home care.
• Coordinate internal and external staffing needs to ensure clients are seen as scheduled
• Provide care to clients in their home
• Assign clients to the appropriate clinicians
• Maintain an up-to-date roster of active and PRN staff to secure last-minute coverage
• Communicate with caregivers regarding any updates or changes to their schedule
• Communicate with managers and clients to inform them of any schedule or staffing changes
• Assist in the hiring, training, and management of new staff

Qualifications

• Professional License Certified Nursing Assistant ( CNA)
• 13 years scheduling or administrative experience (home health, hospice, or postacute care preferred).
• Supervisory experience, reliable attendance; ability to work flexible hours and be part of an oncall rotation.
• Excellent communication skills, customer service and interpersonal skills
• Proficient with scheduling software, Microsoft Office, and other computer programs.
• Strong organization, phone communication, and problemsolving skills.
• Able to multitask and manage multiple priorities.
• High school diploma required; associates degree preferred.
