Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides Person Centered care and services./ppRoles and Responsibilities:/pulliAdministers medications and treatments under the supervision of a Registered Professional Nurse in accordance with Facility Policy./liliTaking the patients vital signs such as respiration, temperature, blood pressure, and heart rate and reporting changes in the patients /residents conditions/liliPractices ethically and professionally within their scope; advocates for patient/liliMaintains clinical competence specific to their care area including organization required education within stated timeframe/liliCoordinates care with interdisciplinary team/liliDocuments appropriately and thoroughly in EMR/Point Click Care/liliKeeps other nursing personnel and responsible RN informed regarding changes in condition of patients/ residents and / or other pertinent observations/liliParticipates in unit councils and staff meetings./liliWeekend rotation required./li/ulpRequired Skills and Experience:/pulliGraduate of a Nursing Program/liliCurrent NYS LPN License/liliCPR/BLS/li/ulpPreferred Skills and Experience:/pulliPrior patient care and customer service experience/li/ulpPhysical Requirements:/pulliMay require lifting (up to 50 lbs.), may require sitting or standing for long periods.
