Licensed Practical Nurse (LPN)
The primary functions of an LPN include assisting with assessment, planning, implementing, and evaluating the care of all patients during the shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides person-centered care and services. Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with facility policy. Takes patients' vital signs such as respiration, temperature, blood pressure, and heart rate, reporting changes in patients' conditions. Practices ethically and professionally within their scope and advocates for patients. Maintains clinical competence specific to the care area, including required education within the stated timeframe. Coordinates care with the interdisciplinary team. Documents appropriately and thoroughly in electronic medical records. Keeps other nursing personnel informed regarding changes in patients' conditions and other pertinent observations. The staffing ratio for this shift is 2 LPNs to 3-4 CNAs for every 40 residents. Typical orientation period is 1 day on the assigned unit. Scrubs of any color may be worn by the employee.
