Step out of the high-volume clinic and into a role where you have the time to actually move the needle for your patients. As a Home Health PT, you are the architect of recovery, helping patients regain independence in their own environment with total control over your treatment plans.

Documentation Efficiency: Our EMR workflow is designed to be completed bedside, ensuring your work ends when your last visit does.
Stable Referral Pipeline: Consistent patient volume across Riverside County—your schedule stays as busy as you want it to be.
Supportive Operations: We handle the scheduling and intake hurdles so you can focus on functional outcomes.
Impact & Responsibilities
Lead evaluations, reassessments, and goal-oriented therapy visits.
Design individualized plans to improve mobility, strength, and safety.
Educate caregivers on home exercise programs (HEP) for sustainable recovery.
Collaborate with an interdisciplinary team to ensure holistic patient progress.
Qualifications
Active California PT License (Required).
Home Health Experience preferred (but we welcome skilled clinicians looking to transition).
Road Ready: Valid DL, reliable vehicle, and auto insurance.
Communication: Ability to clearly bridge the gap between clinical goals and patient motivation.
