Work Environment

This full-time position is onsite and requires travel to patient homes within Danville, providing a unique opportunity to engage with patients in their own environments. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, assessing their needs and establishing personalized goals and treatment plans in line with physicians' directives.
• Deliver individualized therapy sessions focused on improving patient functionality and mobility through evidence-based interventions.
• Maintain accurate and detailed records of patient care, documenting evaluations, treatment progress, and discharge summaries with precise professional terminology.
• Supervise the activities of Physical Therapy Assistants (PTAs) and support personnel, ensuring the provision of high-quality patient care while fostering a collaborative team environment.
• Engage with interdisciplinary team members to coordinate patient care, promoting effective communication and innovative approaches to treatment.
• Provide guidance and training to support staff, assisting them in achieving optimal outcomes for our patients.
• Regularly assess and document the effectiveness of treatment interventions and adjust care plans based on patient progress and feedback.

Patient Population / Client Focus

Your primary focus will be on a diverse patient population, including individuals recovering from surgical procedures, those with chronic illnesses, the elderly, and patients experiencing mobility issues due to various conditions. You'll play a key role in skillfully navigating the complexities of home care, adapting treatments to meet the specific needs and environments of each patient.

Qualifications

• Bachelor of Science degree in Physical Therapy from an accredited university.
• Current licensing as a Physical Therapist in the state of Virginia.
• At least two years of clinical experience in a Physical Therapy setting, preferably in home care or similar environments.
• Strong verbal and written communication skills, enabling effective interactions with patients and team members.
• Proficiency in electronic documentation systems and basic computer software for patient management.

Preferred Experience

• Prior experience working in home healthcare, rehabilitation, or outpatient settings.
• Knowledge of therapeutic modalities and techniques relevant to a wide range of patient needs.

Compensation and Benefits
