Community Housing Innovations (CHI) seeks a dedicated Case Manager to support residents by developing and updating service and care plans, coordinating direct services, and linking residents to community resources. You will monitor progress, collaborate with social workers, and assist with benefits enrollment while maintaining accurate documentation.
