What You'll Do

As a Physical Therapist, you will play a vital role in delivering comprehensive, patient-centered care. Reporting to the Rehabilitation Team Lead, you will:

Perform comprehensive evaluations, assessments, and diagnoses
Develop and implement individualized care plans tailored to each patient
Provide hands-on therapeutic interventions in a home care setting
Educate patients and families to support long-term health and recovery
Collaborate with a multidisciplinary team, including specialty programs such as:
- Parkinson's Disease care
- COVID-19 recovery
- Post-acute stroke rehabilitation
Respond to changing patient needs with clinical judgment and adaptability

What You'll Bring

Education

Bachelor's degree or higher from an accredited Physical Therapy program

Experience

At least 1 year of experience in acute care, rehabilitation, or a related setting
Home care experience preferred, but not required

Licensure & Certification

Active Massachusetts Physical Therapist (PT) license
BLS certification (required within 30 days of hire)
