About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Home Health Experience or 1 full year Hospice Experience depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS within the last 6 months. Description: Provides, coordinates, and directs home nursing care according to physician's orders based on agency policies and procedures. Applies the nursing process competently. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity has a different point value, equating to roughly 30 points per week. Contractor will be assigned between 5-10 patients per day. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is required. Travel up to 75 miles from the home clinic is necessary. Mileage is reimbursed at the standard IRS rate. No guaranteed hours; contractors must meet weekly productivity points to sustain financial needs. Documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation may result in cancellation of contract. Acceptance of all scheduled patients regardless of distance is expected. Must be proficient in HomeCare HomeBase and OASIS.
