Participant Services Strategist
Jobtailor
Jobtailor is seeking a dedicated professional to manage a caseload within our CFSS program in Minnesota. You will assess participant goals, develop Service Delivery Plans, educate on service models, and coordinate care with agencies to ensure compliance and quality. Strong budgeting, documentation, and communication skills are essential. You will collaborate with staff to ensure SDP adherence, address ADLs/IADLs, and provide resources to promote participant independence while maintaining HIPAA #J-18808-Ljbffr Jobtailor
