Coordinates and manages multidisciplinary services to ensure continuity of care, including admissions, transfers, and discharges, and collaborates closely with the interdisciplinary team. Educates patients and families on health maintenance and disease prevention, completes timely electronic documentation in Homecare Homebase, and offers guidance and emotional support throughout the care journey.Qualifications:Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of VirginiaAt least one year of home health experience preferredGraduate of an accredited nursing school.Must have a valid driver's license and a reliable car available during work hours.OASIS and HCHB experience PLUS!If you are interested and are a caring, compassionate nurse who can make a difference in the lives of our patients, then we would be interested in meeting you!"James River is an equal opportunity employer.
