As a Care Coordinator at Aspire Health Partners, your job purpose will be to provide a variety of essential functions to coordinate care for program participants. These functions include: Providing resource linkage, connection and advocacy to program participants. Maintaining up-to-date knowledge of area resources and how to access them. Working with multi-disciplinary teams to gather information and provide the best wraparound care possible for clients. Documenting clearly and effectively in the Electronic Health Record. Following all program specific protocols in relation to specified role, maintaining fidelity to program model. Maintaining all required credentials necessary to perform job functions. Qualifications Bachelor's Degree in Social Services 1 year working with target population required for some positions Level II Background clearance- All Aspire Health Partners Internships and Careers require Level 2 clearance, with Aspire covering fingerprinting costs.
