In-Home RN Case Manager Field Care & Coordination

Cnscares in Lenoir City, TN, is seeking an experienced RN Case Manager for a full-time, Monday-Friday role with on-call rotation. You will oversee coordinated care for a patient group and supervise related clinical activities in-home.
The ideal candidate holds an RN license with an associate or bachelor’s degree, 3 years RN experience, and at least 1 year in case management. Benefits include medical/dental/vision, 401K, HSA, unlimited PTO, and mileage reimbursement.
